of these documents on file before you can retire, so please
submit them on or before your retirement date. Failure to submit
the needed documents may delay the processing of your retirement
benefits. You can provide ACERA with many of these documents at
any point during your career.

If your current name doesn’t match your birth certificate.

If You Divorced or Dissolved Your State-Registered Domestic
Partnership During ACERA Membership
Domestic relationship order (DRO), joinder, and/or
notice of termination of domestic partnership
If You Are Enrolling in Healthcare Coverage, You Must Provide:
Benefits confirmation statement/letter
If you’re retiring from active work for one of ACERA’s participating employers. The
statement should reflect who is currently on your employer
medical, dental and vision plans. Your employer provides this
statement to you annually after open enrollment closes. You can
request an additional copy from your employer’s benefits
department, or from the Employee Benefits
Center if you’re an Alameda County employee.
COBRA Proof of Coverage
If you’re retiring from Deferred membership and you’re planning
on enrolling in ACERA healthcare plans at retirement
immediately from active work.
Signed Medicare card With Part B
If you are Medicare-eligible and planning to enroll in an ACERA
medical care plan. If already enrolled or in the process of
enrolling.
If Enrolling Children in Healthcare Coverage, You Must Provide:
Children’s birth verification
If you plan to enroll your dependent(s) in medical, dental,
and/or vision coverage. See below for acceptable documents.
Children’s social security card
If you plan to enroll your dependent(s) in medical, dental,
and/or vision coverage
Health care dependent summary or open enrollment
statement
If you plan to enroll your dependent(s) in medical, dental,
and/or vision coverage. You can obtain a copy from your
employer’s payroll department or from the Employee Benefits
Center if you work(ed) for Alameda County.
Letter from physician stating disability occured prior
to age 26, or other disability certification
If child is age 26 or older

California or other U.S. State. California REAL ID has
gold bear in upper right corner.
- Certified birth certificate
- Official report of birth abroad
- Official US passport or official passport issued from a US

Not Acceptable as Birth Verification
- Driver’s license or California identification without the
gold bear in the upper right corner. This is the old-style ID
which says “Federal Limits Apply” in the upper right corner.
